New Patients

Your first visit to Robert H. Cornfield, D.P.M., PC, with Dr. Cornfield establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give us time to assess your condition.   If seeking a second opinion please provide as much information including other diagnostic test/images/reports.  If you are an athlete or runner, please bring your running shoes or other athletic foot wear.   Diabetic patients should bring a copy of the most recent Hemaglobin A1C result, and current information pertaining to diabetes management.

PLEASE ARRIVE FOR YOUR SCHEDULED APPOINTMENT 30 (THIRTY) minutes prior to the time you were given.  IT IS ESSENTIAL THAT OUR FRONT DESK/RECEPTION STAFF has adequate time to obtain your personal information.  Yes, we do try to keep all of the necessary paperwork to a minimum  but there are government regulations that must be met and in order for you to be seen in accordance with the time given for an appointment this paperwork must be completed BEFORE you are able to be seen.   WE DO NOT LIKE TO KEEP ANY PATIENT WAITING YOUR HELP IN MAINTAINING OUR TIMELY APPOINTMENTS IS ESSENTIAL TO THIS PROCESS. 

We thank you in advance for arriving 30 minutes before your appointment as a New Patient, and 15 minutes for established patients.
